Permission to Tryout - Link and Instructions

Permission to Tryout forms are required for any player who was registered with another OWHA-Sanctioned team this season.  This form is not needed for non-OWHA sanctioned teams, such as boys' teams.

The form can be requested any time, but is only valid after April 16th, 2024.
