~ GAMES AND PRACTICES WEEKLY ~ TEAM BANQUET & PHOTOS ~ NOVICE to MIDGET LOCAL LEAGUE ~  TEAMS HOCKEY JERSEY SUPPLIED + SOCKS ~ NO RESIDENCY FEES ~

Teams will be a maximum of 15 skaters and a goalie (2 if available)

 

If you plan to pay online while registering, you will have the option to pay with your PayPal account, Visa, MasterCard or American Express.

If you plan to pay by cheque, please register online and chose the cheque option.  Print the registration form prior to submitting and place a copy of the printed form along with a cheque for $620.00 or if preferred **post-dated cheques in an envelope and mail to the attention of:

**Post-dated Basic Registration Payment Option

If you select the cheque option with postdated cheques please make cheques payable for the following dates. 

4 post-dated payments of $155.00 each on:

April 25th, 2016 or day of registration

May 25th, 2016

June 25th, 2016
July 25th, 2016

Or drop off in the OGHA Mailbox at Rotary Place - entrance beside the Rock Wall just outside the OGHA Office - first mailbox on your left as you enter the hallway. **All cheque payments must be received at time of registration and be cleared prior to the 30th of July. 

 


Players are not considered accepted until the registration process has been fully completed - including payment in full.   Space is limited in some divisions.  When you are registering and the age division that you are registering in comes up indicating that it is FULL players will be held in a wait list until enough players are available to consider the possibility of two teams at a level.
